Identification of the causes of water leak in outlet of bath tube

I hage a water leak in the pipe of the bath tube. The leak is inside the floor of the 2nd floor, and the water has leaked through the ceiling down to the 1st floor. i need a report for my insurance that describes the causes of the leak and an estimate of the cost for fixing the problem.
